If you have been asking those questions for years and getting nowhere, there is a condition you may never have been told about. It is called lipedema, it affects women almost exclusively, and it is not caused by anything you did.
Lipedema is a long-term disorder of fat tissue. The fat itself behaves differently: it builds up in a particular pattern, it hurts, and it does not respond to dieting the way ordinary fat does. That last part is the bit nobody tells you, and it is the reason twenty years of effort has not worked the way it should have.
It usually starts or gets noticeably worse at hormonal turning points: puberty, pregnancy, and around menopause. It often runs in families. It is regularly mistaken for ordinary obesity, and sometimes for lymphoedema, which is a different condition.
Both sides, evenlyIt affects both legs the same way, and often both arms. One swollen leg is something else and needs a doctor.
Out of proportionYour lower half looks like it belongs to a bigger person than your upper half. Buying one size on top and three sizes on the bottom.
It stops at the ankleA distinct cuff or shelf just above the foot, with the feet themselves unaffected. This is one of the clearest tells.
It hurts, and it bruisesTender or painful to press, aching and heavy by evening, and bruises appearing with no memory of banging into anything.
The texture changesUnder the skin it can feel like rice, small beads or pea-sized nodules rather than smooth soft fat.
Dieting moves the wrong bitsYou lose from your face, chest and waist while your legs stay exactly as they were. Every single time.
This is not a diagnosis and this page cannot give you one. If several of those describe you, that is a good reason to book an appointment and say the word "lipedema" out loud to a doctor. Ask for a referral to someone who knows the condition, a vascular or lymphatic specialist, or a lipoedema clinic. Take a list of your symptoms with you.

You were not lazy, you were not weak, and you were not making it up. You were doing the right things to the wrong problem.

About that photograph, because it deserves saying plainly. Putting a lean woman in a bikini on a lipedema page is a risky thing to do, and if your first reaction was a small sinking feeling, that is fair and we would rather name it than pretend it did not happen.
It is not there to tell you your legs will look like that. Lipedema varies enormously from woman to woman and nobody can promise you a body. It is there because Carla is 45, she has this condition, and she is on a beach rather than hiding under a towel. That is the part that is available to you, and it does not take until your forties to get it.
You have been promised things before. So here is the line, drawn where the evidence actually puts it rather than where it would sell best.
